Welcome! Many parent resources are available below for both new and returning students. If you do not find the information you need on our web site, feel free to call our offices.
Items Supplied by Parents:
Parents are asked to send a change of outer and under clothes including socks, diapers, labeled bottles with caps, and blanket (or beach towel) for naptime. All items should be labeled with your child’s name. We ask you not to allow your child to bring toys from home because they may create jealousy and tension among the other children. ABC Learning Center, Inc. is not responsible for any items brought from home.
Children will spend time indoors and outdoors daily. Please dress your child in washable play clothes, suitable for all types of activities including painting and eating. Closed-toe shoes with at least a strap on the heel. Shoes must be worn at all times. We ask that you label all apparel. Children should have full change of clothing in their assigned cubby.
Authorization for Child’s Pick-up:
Each child enrolled will have a “Pick-up Authorization” form on file, indicating those individual (must be over age of 18) authorized to remove the child from the center. Exceptions will only be made per the parent/guardian’s report. Such special authorization may be verbal and/or written and must be given prior to the actual pick-up time. The special authorization will be valid for that date entered, therefore; prohibiting continued pick-up privileges (unless their name has been added to the child’s pick-up authorization form).
Every child in the program is required to have a health form, (HRS-H 3040), on file that has been signed by a private physician or a doctor from the health department. This form must be turned in to the Center Manager within one (1) moth after your child starts child care.
This form must be renewed every two (2) years for your child’s safety as well as the safety of the other children.
An up-to-date immunization record (HRS Form 680) is also required and must be provided upon enrollment. Written documentation-stating immunizations are being obtained to comply with this requirement may be submitted in lieu of the form until such time as the HRS Form 680 is submitted
Failure to maintain current health records on all the children in the provider’s care may jeopardize the center’s license. Therefore, it is very important that parents help keep these records up to date. Failure to provide proof that the health and physical records have been up-dated on or before the indicated deadline will result in SUSPENSION for up to five (5) days, followed by TERMINATION.
