Enrollment

Application Procedure:
Parents are required to complete an enrollment application. Parents will receive a parent handbook on the first day of enrollment. The enrollment application will include:
  • Emergency Contracts and/or Alternate pick-up authorization,
  • Parentís Agreement,
  • Emergency Consent,
  • Photograph & Video-Tape Consent,
  • Permission for Medication,
  • Field Trip and Activities Participation Permission,
  • Discipline Policy, and
  • Health Questionnaire.
On our child first day, the parent must complete a short form before leaving the child at the center.
For your convenience, we have provided parent handbook and application form on our web site to download and print.
